All 7th-12th grade student-athletes are welcome to participate in pre-participation sports physicals for the 2018-19 school year on Monday, April 30. Crowley ISD requires ALL student-athletes to have a physical evaluation performed by a physician EVERY year prior to athletic participation. NO physical activity will be allowed without a current physical on file.

Crowley Pre-Participation Sports Physicals

Time: 3:30 - 4:30 p.m.

• Permission forms available in front office; student must have parent consent to attend
• Clinicians provided by: The Orthopedic and Sports Medicine Institute; Dr. Michael Boothby M.D., PA - Team Orthopedic for Crowley High School
• All proceeds benefit the Crowley Sports Medicine Program

North Crowley Pre-Participation Sports Physicals
Date: Monday, April 30
Time: 4-7 p.m.
Cost: $10 (Cash Only)
Location: NCHS Main Gym

Other Notes: 
• All 7th through 12th grade student-athletes and JROTC members are welcome to participate.
• Admittance will not be allowed past 6:45 p.m.
• Concessions will be provided by NCSABC
• Clinicians wil be provided by: The Orthopedic and Sports Medicine Institute; Dr. Michael Boothby M.D., PA - Team Orthopedic for North Crowley High School.
• A short wait time may be necessary to ensure quality exams by clinicians. 
• All proceeds benefit the North Crowley Sports Medicine Program.
